Open this document for guidelines on what is needed to prepare for posting the Pastor's Annual Report for District Assembly.

Annual Pastor's Report

The Annual Pastor's Report is to be completed through the Internet-based "Annual Pastor's Report" software. The address for the software is http://apr.nazarene.org.  You will need to get a User Name and PIN in order to log into this site. The login information has been emailed to all pastors. If you need the login information, please send an email to the District Secretary, Ray Easley at reasley@wbs.edu.

Click HERE if you want to download an electronic copy of the Annual Pastor's Report. You can use this as a worksheet in preparing for completing the report online.

Worksheets for NMI, NYI, and Sunday School can be downloaded below--hand these to your department heads for data that will be needed in completing the Annual Pastor's Report.

Provides the names of Ex Officio and Elected delegates to the District Assembly.

2008 Church Directory (Word)

2008 Church Directory (PDF)

Gives the names and contact information for all of the elected and appointed officers of the local church. This is the important information that will be published in the District Journal.

Deceased Church Member

Allows the local church to send the names of those church members who have passed away since the last District Assembly.

Application for Minister's License

Offers church members the opportunity to apply for District Minister's License. This application is required of all Licensed Ministers (non-Elders) who wish to have their licenses renewed for the coming year.

Ordination/Recognition Questionnaire

Gives a person who desires ordination the opportunity to apply to the Board of Ministerial Credentials and the District Assembly for that elected office. This form is also to be used by any ordained elder of another denomination who wishes to have their ordination recognized by the Church of the Nazarene.

Application for Certification of Role of Ministry

Provides opportunity for any layperson to become recognized for specialized ministries within the local church.

NYI

Sunday School

Allows the local Sunday School, NMI and NYI leaders to compile statistical reports for the Pastor. These forms provide data that will be used in the Annual Pastor's Report. A copy of each should be completed by the respective organizational leaders.

Recommendations

Provides space for the Local Church to pass to the District Assembly a recommendation regarding Minister's (new or renewal), Deaconess, Evangelist, Christian Ed Director, and other laity licenses.
Ordained Minister's Annual Report Gives the non-pastoring ordained minister an opportunity to fulfill his or her reporting obligation to the District Assembly.
Retired Minister's Annual Report Gives the retired minister an opportunity to let the District Assembly know of his or her ministerial activities during the past year.
 


By clicking on the link to each form, you can access a Word document that contains information we need to prepare for the 2008 District Assembly. Please download and complete the two REQUIRED forms. Use the OPTIONAL forms as needed.

You can submit your completed forms in one of two ways:

  1. Save the form to your computer and type the information in the Word document on your computer. Send the completed form as an attachment to an email to the District Secretary (Ray Easley: reasley@wbs.edu). This is the preferred method of delivery.

  2. Type the information on a paper copy of the form and then "snail" mail the form to the District Secretary (see mailing address below).
